Oblong format, 217 x 125 mm. Single sheet. Music copied on recto only; unused ruled staves on verso. No early bookplate. Bound into Mus. 1141d, an early 20th-century guardbook of stiff card and vellum.

Provenance unknown; presumably from either the Aldrich or the Goodson bequests.
