Provenance: bought at auction in Oxfordshire in 1992 (in a lot comprising the items now designated as Mus. 1287-91). Earlier provenance undocumented.

Upright format, 217 x 160 mm. Unbound, the sections largely glued together, as if removed from a tract volume. Lower wrapper (detached) is C17th, with pen-trials on the verso. Upper wrapper (also detached) is modern, dated '21:x:[19]14' in pencil, and with details of the title and imprint in ink.
